Important Information about Patient Review Instrument (PRI)

If you’re being discharged from a hospital and plan to rely on DSS for housing, you must have a PRI (Patient Review Instrument) — without it, DSS will not provide housing assistance. What is the PRI form? The PRI (Patient Review Instrument) hospital release form is a document used primarily in healthcare settings to assess a patient's needs before they are discharged from a facility. This form helps evaluate the patient's medical condition, functional abilities, and any required follow-up care or services upon their release. It typically includes information about the patient's diagnosis, treatment received, and recommendations for continued care, ensuring a smooth transition from inpatient to outpatient care and facilitating appropriate community resources.

A Common Mistake We See

One of the biggest challenges we notice among individuals experiencing homelessness is how they use their limited monthly income. Many people living in cars or on the streets head straight to a hotel as soon as they receive money, only to run out of funds within a few days and end up back outside. We always encourage them to plan carefully — save those hotel stays for when they’re truly needed, such as during severe weather or intolerable storms. Using resources wisely can make a big difference in staying safe and financially stable throughout the month.

🧊 Code Blue Housing Law: What It Means for Suffolk & Nassau Counties

What Is Code Blue? “Code Blue” is a New York State mandate that protects individuals experiencing homelessness during dangerous winter weather. When the temperature (including wind chill) is forecast to drop to 32°F or below for two or more hours, local social service agencies must ensure that no one is left outside. Regardless if your sanctioned or denied services from Social Services Under 18 NYCRR § 304.1, both Suffolk County and Nassau County are required to activate emergency shelter plans during a Code Blue event — removing intake restrictions and providing safe, warm places for anyone in need. What Happens During Code Blue Emergency shelters and warming centers open to all. No one can be denied shelter because of eligibility, sobriety, or identification issues. Transportation is provided when necessary to reach a shelter or warming center. Extended hours of operation may be implemented to ensure overnight protection. Street outreach teams are mobilized to check on people living outdoors and offer assistance. Why It Matters Cold weather can be deadly for individuals living outside or in vehicles. Code Blue ensures that everyone — regardless of situation — has access to warmth and safety. Outreach workers, community organizations, and concerned residents are encouraged to spread the word and check on anyone who may be unsheltered when temperatures fall below freezing. What You Can Do If you see someone in need, call the hotline listed above to get them to safety. Maureen’s Haven is an awesome shelter that will give you shelter along with feed you dinner. They are fully operational November 1st through to April 31st every year. They safely shelter you in one of many churches. From 4pm to 7am nightly. Call them and ask if they have a bed available. Nassau County 866-WARM-BED

The number for WARM BED (1-866-927-6233) is a winter emergency housing program for Nassau County that operates from December 1st through March 31st. This number is for those who are not eligible for other emergency housing programs, often due to not meeting a 30-day residency requirement.

Salvation Army

This is an excellent program. Please be aware that it is a one year faith based homeless and homeless with substance abuse problems program. Felons and undocumented homeless are also accepted. Age limits are18-65 years old.  First two weeks you will have no contact with anyone outside the facility, no phones either. the first two weeks. The Chafee Funds Program - Beyond (CFP-B) of New York State through the Office of Children and Family Services offers limited financial awards to eligible youth who are currently or were in foster care. Pax Christi Hospitality Center in Port Jefferson is a 25-bed emergency shelter for men over the age of 16. The center provides not only shelter and food, but also counseling, advocacy, and networking needs.  They also provide hot showers for anyone in need, along with hot meals for all. Many undocumented immigrants who wish to return to their home country can now use the CBP Home Mobile App, an official U.S. Department of Homeland Security tool designed to make voluntary self-deportation easier and safer. The app is fully available in Spanish and offers several benefits: - Free travel arrangements back to the immigrant’s home country or another country where they have legal status - Forgiveness of any outstanding fines related to illegal presence in the United States - A $1,000 exit bonus to support the transition back home How to Use the Program: 1. Download the CBP Home Mobile App (available in Spanish) on your smartphone. 2. Follow the instructions to apply for voluntary self-deportation. 3. Receive assistance with travel arrangements and benefits from the program. Who Can Apply: - Undocumented immigrants currently in the U.S. who want to return to their home country voluntarily. Additional Support in Long Island: - The Long Island Immigration Clinic (LIIC) offers legal aid and advice for immigrants, including help with asylum applications, work authorization, and understanding options available.
